Public Flu Clinics 2021
The Stratford Health Department will be offering flu clinics this fall. All ages are welcome.

To ensure a safe and timely experience we are encouraging individuals and families to complete their vaccination forms ahead of time and bring a PHOTOCOPY of each individuals license (if applicable) and insurance cards with them to the clinic.
The required forms can be found below. Please print and complete the consent form and bring all completed forms with you to the flu clinic.

PLEASE NOTE: The FluMist Nasal Spray and the High Dose Vaccine WILL NOT be available at the Stratford Health Department flu clinics this year. We are offering Flublok in place of High Dose. What is Flublok? Learn more here.
Vaccines will be given on a first come, first served basis. All clinics are subject to vaccine availability and supply.
Insurances accepted for payment for the flu vaccine include: Medicare Part B, Anthem BC/BS, ConnectiCare, Aetna and Cigna.
WE DO NOT ACCEPT UNITED HEALTHCARE OR OXFORD INSURANCE
All others will be charged a fee payable by cash or check:
Standard Flu Vaccine for all children 6 months to 18 years of age who qualify for VFC: Free
Standard Flu Vaccine for those 18 years and under: $35.00
Flu Block Vaccine for those 18 years and older: $70.00
